House informs his team that Julie is now suffering from a high fever, vomiting and blood-shot eyes. He claims that she has smallpox, caused by the scabs of infected slaves that were kept in the jar as a form of inoculation. With the entire family now held in quarantine, the medical team don protective suits to vaccinate them for smallpox. Julie is now seriously ill, and while Taub attempts to convince a panicked Lulu that her daughter could simply have tetanus, Masters admits that she could have contracted smallpox.
House becomes curious when Cuddy allows him to skip his required clinic hours and contacts Wilson, who is attending to a 6-year-old chemo patient named Eve. Eve wants to be reunited with her stuffed toy before agreeing to the treatment, so Wilson volunteers to have a nurse watch the child while her mother fetches the doll. He then meets with House, who suggests that Cuddy knows about his previous deception involving the lab test. Wilson warns House that if Cuddy doesn't know, his "sniffing around" will make her suspicious. Meanwhile, Taub's tests reveal that Julie's white cell count is depleted. House suggests that increased pressure caused by Julie's recent diving could have forced her antibodies into her joints.
House visits Cuddy's office and she admits that she knows about his deception. He argues that his actions saved a patient's life and that it was not a personal slight against her, but Cuddy remains angry and orders House out of the room. Elsewhere, while performing joint taps on Julie, Taub notices white patches on her leg. Suspecting that she does indeed have smallpox, he decides to contact the CDC. Later, Masters inspects Julie's skin, searching for dark purple blotches that would indicate a less harmful form of the disease. Instead, she discovers a poison ivy-like rash under Julie's arm, which leads House to declare that Julie does not have smallpox.
